To Register:

• Fill out and return the Registration Form now
• Fill out and return the individual registration forms
• Send in the team payment of $1900 which covers:
  • player registration
  • insurance
  • referee fees
  • field usage fees
  • league administration

Registration and Payment Deadline:     August 20, 2010

Season Dates: September 11, 2010 thru November 14, 2010

Roster freeze date: October 15th (last day to add new players)

• Team Registration Form
• Player registration/release forms (signed and completed by each player)
• Team Registration Fee (payable by cash or check only) - includes coverage for USASA player cards and insurance, as well as game and field expenses. The entire Team Registration Fee must be received with registration.
