Competitive Player Uniform Ordering

If you are a BUSA Competitive Player, you need to use one of the following uniform order sheets:

For all Academy or Elite players click here:  Development Academy and Elite Uniforms

For all Select or Premier players click here: BUSA Select & Premier Uniforms

Fill out the appropriate sheet with:

• Player's Name
• Team Manager's Name
• Age Group / Team Name
• Requested Player Number (agreed upon with your team manager)
• Boys or Girls (very important)

For the kit put a check mark in the box for their size of each item.

For any "optional" items put the quantity and in the appropriate "size" box.

Fill in the total amount of your order and return to your team manager with a check made out to your team fund in the appropriate amount.

Team Managers must turn in the team order no later than June 20th, so we have to get these done quickly.  Thank you for your support.
